Everything Simone Biles Has Revealed About Her 3 Plastic Surgeries (and the 1 Enhancement She’ll ‘Never’ Do Again)

Simone Biles has “no shame” when it comes to her elective surgeries.

After hinting at three cosmetic procedures in an October 2025 TikTok, Biles opened up to PEOPLE a month later about her breast augmentation, lower blepharoplasty and earlobe surgery. She also shared why being upfront with her fans felt so important.

“It’s just who I am, and I feel like I’ve always tried to be open, honest and relatable,” she said. “Seeing me win medals at the Olympics, that’s not relatable. But what is relatable is how we feel about ourselves, what we talk about, what we go through and how we share openly, honestly.”

Biles added that there’s “no shame” in going under the knife, saying, “young girls have the right to their own choices.”

Biles has undergone three plastic surgeries and used Botox

Biles has officially joined the ranks of celebrities who have undergone plastic surgery — and been outspoken about it.

In a November 2025 interview with PEOPLE, the Olympian revealed the three procedures she’s had done: a breast augmentation, a lower blepharoplasty and earlobe reconstruction. Biles explained that she spoke out to keep things real online.

“I feel like nowadays with social media, you see everyone and you’re like, ‘Oh, my god, how does she look so good?’ Social media is not real, so that’s why I try to be as transparent as possible,” she shared.

Biles has also tried Botox — but after her first experience in March 2024, she decided it would be her last.

She had a lower blepharoplasty and earlobe surgery

For years, the Olympic gymnast felt self-conscious about the bags under her eyes, a feature that often drew comments from photographers, she said.

“Every time I would go to a shoot, they’re like, ‘Oh, can we put some [eye masks] on her?’ And I was like, ‘Oh, baby, I had 12 hours [of sleep] last night, these aren’t going away. These are just hereditary,’ ” Biles shared with PEOPLE. “So it’s something that I wanted to fix.”

To address what she called the “Biles eye bags,” she underwent a lower blepharoplasty, a procedure that reduces bagginess under the eyes and removes excess skin from the eyelids, per the Cleveland Clinic.

Additionally, Biles’ decision to have earlobe reconstruction surgery came after a childhood accident where one of her earrings “got ripped out.”

Biles has been open about her breast augmentation

In November 2025, Biles confirmed to PEOPLE that she had a breast augmentation. Still, she made it clear that the procedure wasn’t because she “didn’t like” how she looked beforehand.

“It’s just something that I notice[d], obviously a little bit more because I live in this body,” she said. “It’s [all about] feeling good about yourself and loving yourself, and I’ve always been very vocal about that.”

She got Botox on her 27th birthday

Back in March 2024, Biles decided to give Botox a try for the first time, but the results weren’t what she expected.

“I have a funny story for you guys. I got Botox, like, baby Botox, just right here in my T-zone, and I did not like it. So I haven’t got it again,” she told her followers in an August 2024 TikTok.

Biles went on to share how one of her eyebrows would randomly lift on its own during gymnastics practice — something her teammates often pointed out — and she couldn’t get it to settle back down for nearly 20 seconds at a time.

“Not doing that again,” she added of the cosmetic procedure.

Biles later reflected on the experience while speaking to PEOPLE, referencing her “floating eyebrow” and encouraging women not to feel pressured to “fall in line” with what others are doing to their bodies.

“As long as you love yourself, that’s really all that matters,” she continued.
